Clarify ambiguity between 'streams' and 'files'.

We want to general guidelines for stream-level encapsulation to
apply to multiplexed as well as degenerate streams. 'Files' can
therefore specific to the mine-type discussion for unmultiplexed
Ogg physical streams.
