pulse plugin add client_name option

Implement standard "client_name" option.

This allows clients that open multiple streams to give them meaningful names. This includes clients using the python binding, that otherwise will show the client executable name of e.g "python27"

My application for this is the pytone jukebox, that has a main and a secondary (cue) player. I want to move the secondary player to a secondary usb output.
