First cut of full deblocking (R)DO

Performs a near-exhaustive-equivalent search of sse-optimum deblocking
level setup at speed <= 3.  Currently always uses SSE, psychovisual
(cdef-dist) comes next.

Above speed 3, levels are set by quantizer.
